All prospective student-athletes for the 2012-2013 seasons must fill out and return the following health information and return it to Suffolk University Athletic Trainer Jeff Stone to be eligible for athletic participation in the upcoming academic year. All forms must be completed and returned by August 15, 2012.

-Incoming students will also need to take and pass a concussion IMPACT test that will be directed by Trainer Jeff Stone as scheduled by your team.

*This is the information required for all returning student-athletes…the Insurance Info form should be filled out only if your insurance plan has changed over the summer…returning student-athletes do not need a new physical exam, as long as one is on file in the Athletic Training Room.
